Welcome to the Lake Haven Volunteer Application.  Thank you for your interest in volunteering at Lake Haven!  Without volunteers, Lake Haven's rescue and adoption services would not exist! 

 

Use this form if you would like to become a NEW Lake Haven volunteer or if you are a CURRENT volunteer and need to update your contact information. 

 

Please only fill out one form per person.  If you have multiple volunteers in your family, please fill out a separate form for each family member.

 

Volunteers must be 18 years of age or older.  If you are under 18, your parent or guardian MUST volunteer with you.  Parents/Guardians, please fill out a form for yourself and one for each child under 18 who would you would like to volunteer with you.
