Thank you for your interest in volunteering at Lake Haven! Without volunteers, Lake Haven's rescue and adoption services would not exist. It's the dedication of our volunteers that allow us to rescue so many dogs, cats, puppies and kittens.

Volunteer Requirements:

Only those forms that are filled out as defined will be processed.
 
Volunteers must be 18 years of age or older.  If you are under 18, your parent or guardian MUST volunteer with you and complete this form. Parents/Guardians, please fill out a form for yourself and enter the information about your child in the space provided below that is indicated as "Other family members...".

For Dog Fosters  This section is for those who wish to foster a dog. 

In order to place a dog into a new environment we need to know a few things about that environment so we can be sure that the needs of the dog or puppy can be met.

Fostering Commitments  This section is for anyone who wants to foster any animal (i.e. Cat, Dog, Kitten, Puppy).

Medical Needs   

If the foster animal(s) is in need of veterinary attention, shows any signs of illness, or is lost or injured, I agree to immediately contact Lake Haven Rescue.

Who is your current vet? 
We use this information in case emergency vet service is needed and your vet will be used and Lake Haven pays.

All medical treatments must be authorized by the Director of Lake Haven Rescue in advance. I understand that Lake Haven Rescue will make decisions about veterinary care to be provided to the animal based on the prognosis.

I agree to properly administer any required medications to each animal.

I understand that Lake Haven Rescue will inform me of any known medical conditions with the animal(s). However, Lake Haven Rescue cannot be responsible or liable for any unforeseen health problems.

I agree to transport the animals back to Lake Haven Rescue or to the vet for vaccinations, spay/neuter surgery, or other scheduled vet appointments.

In the event of any medical concerns I will contact the Director of Lake Haven Rescue as soon as possible.

I am aware that any dog may bite if provoked so I agree to never allow any stranger or child to be near the animal(s) until it is trusted, and I will never allow another adult or child to threaten, squeeze, poke, step on, or otherwise pose a perceived threat to the animal(s). IF the animal is determined to be a biter or a bite risk, I will contact Lake Haven Rescue immediately.

 

In Home Care   

Everyone in the household knows about, and agrees to, having the animal(s) in the home.

I agree to provide daily food, fresh water, and a litter box (for cats) at all times. I also agree to maintain a high level of cleanliness in their living environment. I also understand that Lake Haven Rescue will provide food, crates, and medicines as needed.

I agree to provide attention and affection to the animal(s) in order to help socialize them.

I agree to secure the animal(s) indoors. Dogs must be on a leash at all times when out of the house. Cats must be transported in a secure carrier.

I agree to provide updates on how the animal(s) is doing should I be contacted by Lake Haven Rescue.

I assume full responsibility for any property damage the foster animal(s) may cause.

I understand that if the care of the animal(s) is to be transferred to any other individual that I must contact Lake Haven Rescue in advance for approval.

 

Returning Animal(s)   

If I am unable or no longer want to care for the animal(s), I agree to contact Lake Haven Rescue immediately. Lake Haven Rescue agrees to accept that animal(s) back upon request.

I agree that a Lake Haven Rescue representatives may remove the animal(s) at any time for veterinary care, adoption, or any other purpose.
